Family Counseling and Consultation Services
- Assessment of client’s current situation and identification of options to address risks and concerns
- Meeting with family representatives to review client assessment
- Private counseling with family members to reduce conflict and to promote cohesiveness and cooperation
- Review of the care options available for your parents; including services needed and location preferences
- Liaison between patient and care givers
- Physician communication, as directed by client
- Review of advance directives with family
Management of client relocation to retirement community or care facility
- Assessment of client’s current situation
- Meeting with family representatives to review client assessment
- Communication with client’s healthcare professionals to coordinate and clarify the clients admission and discharge plan of care when relocation follow hospitalization
- Identification of the care facility or retirement community that best meets the needs of the patient and family
- Coordination of all needed ” therapies”
- Assistance and education regarding Medicare and/or Medicaid coverage rules and benefits to ensure that clients understand their benefits and “out of pocket” responsibilities
- Assistance with billing issues
- Coordination of durable medical equipment purchasing/rental (if needed after hospitalization)
- Assistance with admission process
- Assistance with transition issues
Management and oversight of in-home care
- Meeting with family representatives to assess care needs
- Assistance with activation of “medical alert” services, if applicable
- Hiring, staffing and oversight of private home health aides, if necessary
- Coordination and monitoring of health care related services for client
- Coordination and delivery of medical equipment
- Liaison between health care providers and family
- Coordination of care plans between physicians and assistance with physician communication to ensure that patient and family understand diagnosis and care needs
- Daily communication with client and staff
- 3 to 5 “on site” client visits per week
- Monthly client status review with family
- Conflict resolution with health care providers, if applicable
- Assistance with claim issues and review of medical bills
- Assessment of Advance Directives (Living Wills, Health Care Power of Attorney, Financial Power of Attorney)
Management and oversight of “in-facility” care
- Initial meeting with family representatives to assess oversight needs
- Attend regular care conferences with facility caregivers
- Daily communication with facility staff
- 3 to 5 “on site” visits per week (at non scheduled times)
- Ongoing communication with family representatives to review client status
- Liaison between family and facility
- Conflict resolution
- On-going communication with physicians
Management and oversight of “End of Life” Care
- Facilitation of difficult family discussion; understanding different viewpoints and clarifying wants of the patient
- Support and assistance through the stages of dying
- Hospice service evaluation
- Hospice care coordination
